Re: Jim Michlleton, aged 84

Jim Middleton, who was admitted to the surgery Ward on 7 July 2008 with diagnosed left inguinal hernia, under went surgery of hernia repaire. Requires continue care and support being discharge today.

Post Operative no serious complications were noted. Mr Jim experinced mild pain while ambulating, but has recuperate Completely.

He was diagnosed hypertension in 1998 for which on medication atacand 4mg once a day.

Mrs olgo, Jim's wife,Worried for their financial resources .She think they may eligible to have Pension and other support from the department of veterans

affairs.

upon discharged, advised to keep himself away from heavy lifting. Besides, that driving is not allowed atlease six weeks. Tablet Panadol 2 recommend every six hourly for relieve of pain and light excercise advised . post operative an appointment has been schedule on August at 10Am. In addition, Communicated with the department of venteran veterans affairs for eligiblety of Pension and home support.

Based on aforementioned circumstances appropriate Support of Mr and Mrs Jim would be extremely beneficial
