I am writing to refer Ms Williams, who requires follow up care and health education program for uncontrolled diabetes.

Ms Williams is lives with her husband and socially active person. She is diabetic for the past ten years with non compliance of treatment regime , especially to the diet which includes cream cakes and fruits. She is over weight with less physical activity and gained around 3.5 kgs within 6 months.

Ms Williams had has uncontrolled hypertension for past 5years and on medication. She uses spectacle and her vision is good with it . She prefers to take a glass of wine along with evening meal time.

Upon her treatment plan, kindly provide diabetic education program which includes monitor blood glucose levels with glucometer, importance of diabetic diet, suggest a suitable exercise program to lose weight and vision check-up for once in three months. Encouraged her to participate in classes at hospital in daytime. Of note, her daonil is increased from 15 to 20mg per day.
