Re: Ms Sheila Cartwright, aged 69
Ms Cartwright, who has unstable diabetes mellitus and infected foot ulcer, requires follow-up care and appropriate management followed her discharge today.
On 14 April 2017, Ms Cartwright was admitted for controlling her diabetes. Medications such as glibenclamide 5mg daily, metformin 800 Ms thice daily, along with amoxycillin 125mg twice daily were commenced. Daily dressing with saline was initiated for the foot ulcer.
Ms Cartwright is a retired administrator. She lives alone and no family or friends are available. She has a small amount of pension. She has had diabetes for the past 9 years. Since her retirement, alcohol consumption is high, but dietary quality was poor.Fails to control diabetes with medication.
Upon discharge, monitoring medication compliance such as glibenclamide has to be continued and antibiotics till the completion of the current course. Encouragement of moderate exercise program is necessary. Suggestion of some income- producing activities along with social activities would be appreciated.
Based on the aforementioned circumstances, providing overall lifestyle modification is greatly beneficial. If any additional information is required, please do not hesitate to contact me.
