Re: Alicia Nesbitt, DOB: 22 April 2016


Alicia, baby of Ms Gemma Poole, was diagnosed with low birth weight and opioid dependence, requires follow-up care and support following her discharge today.


Ms Poole, who has been heroin dependent for the past 2 years. She is single and recently worked as a sex worker. Alicia's father alleged domestic violence towards Poole during her pregnancy.


Both mother and baby completed heroin withdrawal without any complications. Social worker involved and approved their discharge arrange long time stay with Poole 's mother. Drug and alcohol team has been involved in managing Poole's ongoing addiction use.


Currently, Alicia has 2.3 kilogram weight.Up on discharge, daily visit is required until the baby's weight and feeding get stable. Ensuring the safe environment for the baby and update the social worker, if any risk has noticed. Education regarding infant care to the mother and grandmother is necessary. Monitoring Ms Poole' s coping and psychological state is important. Please note, liaise with drug and alcohol team to provide integrated support for the mother to reduce the risk of heroin use.


Based on the aforementioned circumstances, providing appropriate management would be appreciated. If any additional information is required, please do not hesitate to contact me.
