Dear Ms Roberts,
Re: Mrs Jasmine Thompson, aged 75 years
I am writing to refer Mrs Thompson, who has undergone a total shoulder replacement. She requires further care and assistance in her daily living after her discharge today.
Postoperatively, Mrs Thompsons vitals were stable. She currently takes clexane and penadine forte for pain relief. Her wound site looks clean and dry. She has been very compliant with the physiotherapy regime.
Mrs Thompson has been instructed to apply cold compression and shoulder brace for next four weeks. And adviced not to lift weights. She has been encouraged to participate in physiotherapy and hydrotherapy for a couple of weeks. She has to come for follow-up in orthopaedic department in 10 days where her staples will be removed.
It would be greatly appreciated , if you could assist her with daily activities and subcutaneous administration of clexane for four days to prevent deep vein thrombosis.
