Re: Ms Susan Millar, D.O.B: 11 July 1992


Ms Susan Millar, who was diagnosed with chronic bronchitis, is being refferred for ongoing care and appropriate management at home from your facility.


On 13 September 2016, Ms Millar was admitted with the complaints of unmanageable cough and dyspnea, despite using over the counter medications. Radiological tests were revealed severe bronchitis, for which oxygen therapy was initiated. She has been treated with anti-inflammatory medications and steroids, along with antibiotics and bronchodilators for the three and five days respectively.

Also, chest physiotherapy and spirometry started which will be continued. Currently, excellent prognosis was noticed with the evidence of repeated X-ray.


Ms Millar who is bachelor, lives alone with poor social interaction. She has been consumes alcohol and smokes for the long time ; therefore, advices for cessation of smoking and alcohol been provided. She has had hypertension since 2012, for which she takes amlodepine 10mg.


Based on the aforementioned circumstances, regular monitoring of medication and steam inhalation three times a day would be extremely beneficial. Visits for physiotherapy at home daily is greatly encouraged. Follow-up visit in the clinic have been scheduled on 1 October 2016. Of note, kindly contact Dr Brown is deemed appropriate.
