Re: Mrs Jennifer Milton, aged 35
Thank you for accepting Mrs Jennifer Milton, who is antenatal mother 25 weeks with G3P2A1L1 diagnosed with pregnancy induced hypertension and gestational diabetes mellitus, is being referred for expert assessment and appropriate treatment from your services.
On 02 July 2019, Mrs Milton was admitted with the complaints of generalized edema and palpitation for the last three days. The vital signs were unremarkable with the exception of blood pressure and pulse. Laboratory results revealed low haemoglobin, which was treated with one unit of packed red blood cells. Despite, taking thyroxin 50mcg, thyroid stimulating hormone levels elevated. Random blood glucose 300mg/dl was observed; therefore, she has been commenced on hypoglycemic agentsalong with regular monitoring of blood and urine sugar levels.
Mrs Milton, weight loss of approximately eight kilograms was noted, for which a referral to ditician was offered for nutritional assessment and dietary modifications. She has been treated for hypothyroidism, she takes Tab: Thyroxin 50mcg. She has had post-partum psychosis and anaemia for previous pregnancy. List of the medication are enlisted with this referral letter.
Based on the aforementioned circumstances, your willingness to evaluate the needs of insulin and increase Thyroxin tablets would be extremely beautiful to enhance Mrs Milton's health status.
