Re: Mr Lionel Ramamurthy, aged 63


I am updating the current health status of Mr Ramamurthy, who was admitted with diagnosis of pneumonia. He requires observation and ongoing care at your facility following his discharge tomorrow.


Upon admission, Mr Ramamurthy required assistance in daily living activities as well as pick-up frame was used for mobilization because of weakness. During hospitalization, chest physiotherapy, deep breathing and coughing exercises was performed therefore, ambulation was reviewed by physiotherapy. To secure the postural drainage, sitting position was recommended than lying down.


On assessment, he was noticed afebrile and inflammatory markers become normal. Also he started doing daily activities without any assistance. Follow-up done with the dietitian and weight was recorded. Overall, he was feeling good.


After discharge, stipulated programs must be performed; dietary regimen must be monitored by providing good nutritional supplements and increased fluids; exercise regimen should be implemented for chest and breathing. Always keep him warm and if necessary paracetamol was recommended.


Based on the aforementioned circumstances, regular supervision and appropriate care would be extremely beneficial to improve Mr Ramamurthy's condition. In case, additional information is required, kindly do not hesitate to contact me.
