Dear Dr Staphem,

Re: Victoria Price, aged 14

27 August 2021, Victoria presented to the School Clinic with the complaint of a traumatized wound of hitting a ball while playing hockey. On examination, multiple, healed clear- cut-scratches on the same hand but at various times were noticed. She revealed that injured from a prickly bush: nonetheless, it seems to be suspected deliberate self-harm. Apart from that, responsive behaviour showed apathy and depression.

Vitoria lives with a forester family since 2012.Today, Mrs Farmer, who rise up her, elaborated her temperamental nature along with introvert personality at home. She often socially isolates and spends the time listening to music in an own room.

At the school, Vitoria is quite disruptive to teachers though she is clever. She also has no close peers and has been distractive to fellow students. Even though sports were her favorite earlier, she is presently showing less interest following her grandmother’s demise. She also does reading excellently whereas, without any curiosity.

Based on the aforementioned circumstances, your urgent attention and necessary treatments would be extremely beneficial to early detect and improve Victoria’s physical as well as psychological status. Foster parent was informed to liaise with the General Practitioner as possible.
