Dear sir /madam,
Re: Ms Patricia Styles; aged 63
I m writing to introduce Ms Styles who has a possible relapse of pericarditis and requires urgent assessment and management.
On today's home visit, she felt tired and had chest pain which was relieved by setting as well as shortness of breath and fatigue. She had a mild grade fever and BP was noted 180/90
On 07/04/2018, she was admitted to Green Valley hospital with chest pain, shortness of breath and tiredness. The throat swab confirmed viral influenza type b and an echocardiogram indicated pericarditis; however chest x-ray was normal. She was commenced on iv saline and antibiotics. She was discharged home on 09/04/2018 and a nurse was arranged for further follow-up.
Medically, she has a history of hypertension and diabetes Mellitus for which she is on regular medication.
Given the above, it would be greatly appreciated if you could take over his care and provide appropriate treatment.
