Dear Ms Ina,

I am writing to refer Ms Dijana who needs continued care and management from your facility.she had external fixation and fasciotomies on her right lower leg and is being discharged today.


Ms Dijana was admitted on 15 July with the diagnosis of proximal and lateral tibial fracture of right leg following a fall. Postoperatively, she was assessed for signs of infection on the wound site as well as wound drainage.she showed good improvement to the treatment.

Ms. Dijana is a school teacher and has two children.she has the history of seizures, for which she is on carbamazepine.


It would be greatly appreciated if you could monitor Ms Dijana ‘s wound for any signs and infection. Kindly teach her regarding signs and symptoms of infection along with expected drainage . Please alert her to inform a nurse immediately if she notice anything unusual on the wound site. It is important to advise her regarding how to keep the area sterile, by keeping blankets and clothing away from wound site. She need adequate rest in order to get a better recovery.
