Re: Mr Henry O’Keefe, DOB: 2 February 1925
Mr Henry O’Keefe, who has been recuperating from malignant melanoma on the left shoulder, requires assistance and support for daily living activities from your services.
According to subsequent visit, satisfactory wound healing without any signs of infection is reported. Complaints of increased arthritic pains in hands legs has been noticed. Subsequently, two Panadol tablets thrice a day, along with Glucusamine and Chondroitin supplements are recommended. Also, troubles with balance maintenance has been observed postoperatively. Regular wound dressing changes are being conducted and assistance in showering is provided.
In addition, Mr O’ keefe lives with her wife, named Dorothy. Concerns related to future care of family has been experienced, as both the members are aged. Troubles in managing kitchen and bathroom accessories have been reported. House is fully disorganised and poorly managed. Mr O’Keefe usually forgets about things, poor memory is noticed in this concern.
Therefore, Mrs O’Keefe requires regular assistance in all daily living activities. Troubles in cooking meals is also observed, so neighbours are helping in meals and shopping.
Based on the aforementioned circumstances, a home visit to assess their needs and appropriate further assistance towards Mr Henry and Dorothy’s social condition would be profoundly beneficial. In case you require any additional information, please do not hesitate to contact me.
