Dear Dr. Dane,
Re: Mr. Timothy Patterson, aged 69
I am writing to refer Mr. Patterson, who has diagnosed with a lower left arm laceration and is suspected to have orthostatic hypertension. He requires full cardiac assessment and management from your facility.
Mr. Patterson has had hypertension since 2018 and is non-compliance with medication and exercise. He also leading a sedentary life style and his body weight is more than the recommended level.
Mr. Patterson was admitted to this hospital on 01 July 2019 with deep wound on the lower arm. During hospitalization, he was initiated with intravenous antibiotics and wound dressing. Subsequently, IV antibiotics was switched to oral antibiotics. Currently, he has made good recovery. His wound is completely healed and there is no sign of infection. However, his blood pressure has been markedly elevated.
It would be greatly appreciated if you could take over the case of Mr. Patterson and provide expert care. Please note that, his blood pressure chart has been attached with this letter.
