The Community Nurse Head

Care Well Hospital

Birmingham

05 May 2010

Dear Head Nurse,

Re: Mr Alfred Billy, aged: 52

Mr Alfred Billy requires to follow up care and monitoring, who was diagnosed with basal cell carcinoma. He is being discharged today.

On 05 May 2010, Mr Billy was admitted to hospital and following his hospital stay, he was undergone for removal of basal cell carcinoma. His skin biopsy has sent for pathological study. Thereafter he was commenced on panadein forte 500 mg for pain which needs to be continued one more week. Presently, he is recuperating well.

Mr Billy is a truck driver and lives with his wife. He has had a habit of smoking cigarettes and consuming liquor for the past 30 years. Apart from this, he has no medical history of hospitalization.

It would be greatly appreciated if you could monitor his medication compliance and progress of wound healing. Please note, Mr Perth is to be contacted in case of emergency.

Yours faithfully,

Charge Nurse
