Re: Ms Riya Joy, DOB: 12 March 1998
Ms Riya Joy, who was a single parent of a new born baby girl, is being referred to your facility for further accommodation and supportive management of care.
Ms Joy was admitted on 21 February 2018, who underwent a vaginal delivery on 22 February 2018at 12.30 hours, was given birth to a baby girl weighing 3.77 kilograms. During the antenatal period, she done six visit and taken folic acid, vitamin D and calcium supplements. Tonsillectomy was done at 14. For the last 1.5 years, she was on depression due to the death of parents and was untreated.
Presently, she is single but lived with the boyfriend from August 2017. Her current boyfriend lives in Europe wereas she is not interested to live with him. Also he is not the father of her child.
During the postnatal period, minimal blood loss was recorded and hemoglobin was 135 grams per litre. Breastfeeding was hesitated therefore promoted bottle feeding and feeds well. Upon discharge, baby weight was recorded 3.95 kilograms. Also provide a brochure regarding single mother benefits. Psychologist consultation was done to provide emotional support as well as counseling regarding parents and boyfriend relationship.
Based on the aforementioned circumstances, regular supervision and ongoing postnatal care would be extremely beneficial to improve Ms Joy's condition. Incase, additional information is required, kindly do not hesitate to contact me.
