Re: Mr. Lionel Ramamurthy, aged 63
I am writing to refer Mr. Lionel Ramamurthy, who has been suffering from pneumonia for one week. He requires continued care and management following discharge today.
Mr. Ramamurthy was admitted to the hospital with complaints of shortness of breath, inspiratory and expiratory wheezing sound, fever, rigors, sleeplessness and gernalised ache. In addition, he complaints of chest and abdominal pain due to persistent cough. On admission, he was commenced only short distances with a pick-up frame to increase shortness of breath on excretion. He also assisted with activities of daily living in showering and dressing.
During hospitalization, Mr. Ramamurthy has been encouraged to chest physiotherapy. Please note, sitting position is comfortable for ensuring postural drainage. He has been encouraged with oral fluids and proper nutrition. Now he gained weight with dietitian’s input.
It would be greatly appreciated if you could keep warm Mr. Ramamurthy and give proper nutrition to him. Paracetamol is prescribed in case of chest and abdominal pain.
