Dear Penny,

Re: Mrs. Sheila Cartwright, aged 69

I am writing to refer Mrs. Cartwright, who was diagnosed with unstable diabetes mellitus and small infected foot ulcer. She requires continued after being discharge today.

On 14/04/2017, Mrs. Cartwright was admitted with above mentioned complaints, for which tablets glibenclaminde 5mg daily, metformin 800 mg (t.d.s.) and amoxicillin 125 mg orally (b.d.) were provided. Daily dressing with saline was done. Wound margins, observe for signs of complications and healing process were monitored. She has had an non- insulin dependent diabetes mellitus since nine years.

Mrs. Cartwright worked in an administrative position and retired at 67. She receives small pension for her daily living. She has been lived alone in own three room bedroom home. However, her alcohol intake has increased and dietary quality has decreased after retirement.

It would be greatly appreciated if you could manage her condition. Medication compliance, blood sugar level and alcohol intake as well as diet needs to be monitored. You should be encouraged for exercise programme and establishment of social activities. Income producing activity should be suggested to her.
