Re : Mr Peter Dunbar

DOB: 18/03/1932


I am writing to transfer Mr Dunbar into your care for his ongoing care and management. He is being transferred to your City with his daughter and her husband.


In 2018 ,Mr Dunbar was hospitalized with the diagnosis of myocardial infarction for which he was treated with sotalol and warfarin along with the hypertensive tablets. He was non-compliance with his diet medications and exercise, inspite of giving education regarding their importance. He was blaming himself for forgetful. He mobilize independently with the support of walking stick.


Mr dunbar's medical history in 2017 was diagnosed with type 2 diabetes which was controlled by diet management as well as supervised by his wife. He was commenced on tablet metformin. Since his wife's death ,he was continued with excessive amount of alcohol ,salt ,sugar and fatty food .

In view of the above details, it would be greatly appreciated if you could provide education regarding monitoring the medications on time and management of his diet to his daughter as she is going to cook for him .
