Re: Mr lionel Ramamurthy, aged 63


I am writing to refer Mr Ramamurthy ,who was diagnosed with pneumonia as he requires an ongoing care at your service . He is being discharged today.


Mr Ramamurthy was admitted to the hospital on 04/02 /2019 with the complaints of shortness of breath wheezing ,while breathing, fever rigors , sleeplessness and generalized pain. He also developed the chest and abdominal ache due to prolonged coughing . During hospitalization, he was weak and had increasing shortness of breath on exertion while mobilizing a short distance with the help of a pick-up frame.


Mr Ramamurthy has made a good progress he is able to perform daily activities of living and walk slowly independently.


Upon discharge, it would be greatly appreciated if you could provide him a healthy diet and encourage him to take oral fluids. Kindly ensure that he is kept warm. Please encourage him to follow his exercise program .Please note that, Paracetamol was prescribed to alleviate the pain as necessary .
