Re: Mr Lionel Ramamurthy, aged 63
I am referring Mr Lionel Ramamurthy, who was diagnosed with pneumonia, for ongoing care and further assistance at your facility, following the discharge back tomorrow.
On 4 February 2016, Mr Ramamurthy was presented to the hospital with the complaints of acute shortness of breath, fever, rigors, wheezing, sleeplessness, chest and abdominal pain due to persistent cough. Additionally, assisting in providing with daily living of activities such as dressing and showering, his shortness of breath had increased while ambulation. Apart from that he is also able to walk but slowly without any assistance, shower and toilet use independently.
Following discharge, paracetamol recommended for chest and abdominal pain according to requirement. Chest physiotherapy for deep breathing and coughing exercises should be encouraged, ensures that he sits up rather than lies down, whenever possible to ensure postural drainage. Proper nutrition and oral fluids are highly suggested. Therefore, Ms Ramamurthy should be kept warm. The condition is currently stabilised.
