Re: Mrs Maria John, aged 60
Mrs Maria John, who has been suffering from hyperthyroidism and hyperglycemia, is being referred for further management and ongoing care.
On 11 December 2014, Mrs John was admitted with complaints of constipation associated with sever abdominal pain as well as joint pain, irritability, dry mouth and slurred speech was recorded. As a result increased thirst was noticed. On examination, blood pressure was increased by 170/110 mm of hg and other vitals were remarkable.
In regards to past medical history, Mrs John has had hernia repair, osteoporosis and hyperthyroidism in 2010, 2011 and 2014 respectively. Also had irritable bowel syndrome, osteoarthritis, seizure disorder, hypertension and stroke was recorded. In 2010, a whole blood was transfused during post-operative period.
During hospitalization, blood sugar level was increased up to 400 mg per dl and TSH, T3 T4 was deranged. MRI and ultrasound sonography revealed slight bleeding in temporal lobe and bulging in right upper quadrant of the abdomen respectively, whereas x-ray shows enlarged glands in the neck. Thyroidectomy was commenced. The list of medications is attached with the letter for your kind perusal.
Based on the aforementioned circumstances, regular supervision and appropriate management would be extremely beneficial to improve Mrs John's condition. Incase, additional information is required, kindly do not hesitate to contact me.
