Dear Ms Mullins,


Re: Ms Pamela Viduka, aged 72 Years


I am referring Ms Viduka, who underwent a mitral valve replacement surgery, requires ongoing care, management and is being discharged today.


On 18 November 2008, Ms Viduka was admitted to hospital with symptoms of chest pain and other associated ailments. After assessment, she was diagnosed with mitral valve prolapse. Therefore, surgery was performed to correct the defect. Post-operatively, tablet warfarin twice daily, antibiotics, and pain medications have been commenced on her. On the Subsequent day, she has been recommended with low salt diet and assistance were provided for shiwering, dressing as well as walking. She has had recurrent chest infection. Hence education was provided to her regarding the prevention of such infection and the importance of smoking cessation. In addition, she was seen by a dietician and dietary advice was provided. Today, excellent improvement of the wound has been noticed.


Ms Viduka is a widower and lives with her daughter. She has had hypertension and chronic bronchitis, as well as a chest infection has been noted for the past 12 years.


Pertaining to the above-mentioned information, kindly take over the case of Ms Viduka and provide continuous care. She is on tablet warfarin. Therefore, regular INR monitoring is required. Avoidance of invasive procedure and monitoring of her medication compliance would be profoundly beneficial.


All medication list is attached with this letter for your kind reference.
