Dear sir/madam,
Re:Mr Stephen kerry, aged 46 years
I am writing to refer Mr kerry into your care who require support and followup care from your service. He is being diagnosed with right knee injury.
Mr.kerry presented to our nursing agency followed by fall from stair on 12 July 2011.He had the history of cerebrovascular accident 2 years ago.Hence he was impaired balance and walking with the limp.Presently,he is recuperating well with no infection around the injury. Mr kerry has been commenced with 400mg paracetamol for pain if necessary. Along with that application of warm compression is also doing with daily dressing.
However,Mr kerry can able to walk by using a walking sticks for support .Mrs kerry requested to have more visiting to increase his mobility and to go to work.
It would be greatly appreciated if you could provide a home visit to help Mr kerry to improve his mobility following his recent knee injury.
