Re: Ms Margaret Helen Martin, aged 81


Ms Margaret Helen Martin, who has undergone a sucessful angioplasty for coronary artery disease and angina, requires ongoing care and assistance from a community health nurse after being discharged tomorrow.


Due to sedentary lifestyle, she is overweight and BMI of 29.5 was recorded. She dose not stepout from the house, due to lack of family interaction as well as social interaction which leads to isolation.


Her ongoing medications include aspirin and Plavix for CAD; Betaloc and Tritace for hypertension; Lipitor for hypercholesterolemia is highly recommended.


Concern regarding adjustment to homely environment was raised, therefore requested for an extention. Providing low-calorie, high-protein, low-cholesterol, gluten-free diet was preferred. Frequent small meals or snacks; excessive intake of fluid was suggested. Based on the recommendations of physiotherapist daily exercises, short periodic walk, engaging personally in activities by applying physical strength is suggested periodic investigations for any signs and symptoms for bruising is suggested. Arrange regular family visit meetings is observed and may help to prevent any feeling of isolation.
