Dear Dr Dsouza,
Re: Maria John, aged 60
Mrs Maria John, who was diagnosed with hyperthyroidism, requires further assessment and appropriate management following the discharge today.
Mrs John has been treated for hypothyroidism for four months, for which she presently takes thyroxin twenty-five milligrams two times a day. She also has a history of hypertension, osteoarthritis and osteoporosis.
On 11 December 2014, Mrs John presented with the complaints of irritability, fatigue, dizziness along with slurred speech and increased thirst. Laboratory investigations revealed that decreasing TSH and increasing T3 and T4 levels. Not only that, a x-ray which was performed showed enlarged glands around the neck. However, elevated blood pressure and sugar levels were managed with anti-hypertensive and insulin respectively. She was anemic and a whole blood was transfused.
Mrs John was advised regarding dietary modifications including calcium supplements. Apart from that, physiotherapy along with regular exercises is suggested, Routine medications should be continued and a follow-up with TSH has been scheduled in a couple of weeks.
Based on the aforementioned circumstances, your expert evaluation and necessary treatment options to improve Mrs John’s health condition would be extremely beneficial.
