Re: Ms Marry Bell, aged 66
Ms Mary Bell, who was diagnosed with unstable diabetes mellitus and minor infected leg foot ulcer, requires regular monitoring and appropriate management following the discharge today.
Ms Bell has been treated for type one diabetes mellitus for fifteen years, for which she takes metformin 850 mg trice and Glimel 5 mg once daily. Augmentin Duo Forte 875/125 mg orally twice a day has been prescribed and review will be needed upon completion. Observation of wound margins and signs of complications is needed.
Since retirement, Ms Bell lives alone and survives on a merged pension. However, her alcohol consumption has drastically increased whereas the diet quality has deteriorated tremendously. Additionally, continuous problems with self-administration of hypoglycemic medications were reported. Therefore, ongoing supervision of medication compliance, gradual reduction of alcohol intake, dietary quality, exercise regimen and blood sugar levels is highly recommended.
Based on the aforementioned circumstances, complete transformation of personal lifestyle would be extremely beneficial for improving Ms Bell’s overall health condition. Apart from that, participating in social activities, establishing income-generated measures and involving in community social worker services will be profitable.
