Re: Mrs Margaret Adam, DOB: 08 May 1945
Mrs Adam, who has been recuperating from left total hip replacement surgery, requires ongoing care and appropriate management following her discharge today,
On 21 July 2021, Mrs Adam was admitted with a fall and was diagnosed a fracture on left hip acetabulum. Postoperatively, delirium and disorientation regarding time, place and person were observed. Medications such as injection paracetamol! gram 8 hourly and if needed along with her regular medication alendronic acid, 70 mg; calcium, vitamin D and deltaparin 500 IU. Physiotherapy was initiated for mobility where as gradual prognosis has been noticed. Self-administration of deltaparin was encouraged, but monitoring is required. Activities of daily living has been performed and can walk a few steps and stairs alone.
Mrs Adam is a widow, who has no close relatives. Her neighbours are very supportive. She coordinates meals delivery service. She has had macular degeneration and blurred vision since 2018. She had an ankle fracture and wrist fracture in 2012 and 2018 respectively. She was diagnosed with osteoporosis in 2020.
Upon discharge, monitoring medications are necessary. Continue regular exercise is important for the prognosis of her condition. Close observation is required for the swelling and pressure ulcer. Gentle reminder for the wound dressing has been scheduled on 06 September 2021, antiembolism stocking on the same day and cessation of deltaparin on 17 August 2021. Ensure the visit at least once in two weeks and medicine revival is on 22 September 2021.
