Re: Mr Peter Dunbar,
DOB: 18 March 1932.
Mr Dunbar is being transferred to your community requires monitoring and encouragement. He has
plan to live with his daughter in your community.
Mr Dunbar has had type 2 diabetes hypertension, atnal fibrillation and on Melformin 500mg three times
per day, Rnipril 5 mg daily Warfarin variable 3.5mg, solatol 40mg daily respectively. Since June2018 he
has been taking warfin and solatol medications for post M1. potential adverse effects of intermittent
dosing several time but still requires reduction he have poor memory.
He has been wrong stick while walking due to diabetic neutopathy sice october 2018, poor excercise
tolerance, restricted mobility were notice. He intaken alcohol.
He was living in tetiremnt village after wife death,now he is planning to live with his daughter. His
daughter requires education regarding needs and monitoring. He is reluctant and resistant to dietary
alterations and medication regimen. Kindly note that, low-fat, low-sugar, colorie restriction is preferred
and also limitedalcohol is advicable. it would be appriciated if you could monitor and encourage him
reqarding diet and medications.
Based on the aforementioned circumstances keep monitoring and do needful.
