Re: Mr Lionel Ramamurhy, Aged 63

Mr Ramamurthy is being transferred back to retirement home after the pneumonia treatment. He is being discharged today.

As you awar he was admitted with this condition symptoms along with fever, rigors , sleeplessness genaralised ache. On admission observed using pick updrame during mobilization and requred assistance in ADLs also shortness of breath on excertion noted.

Subsequently. his inflammatory masks revoled normal beconte independent but doing slow. On examination, dry cough some chest and abdominal pain identified, he gain 1.5 kgs after dietiction review. His health progress is satisfactory

Upon discharge, he is encourged to take oral fluids and propu nutrition and also warn atmosphere. Kindly encourage him to do chest physio i.e deep breating and coughing excercise. Please note that sitting postion is preferred for him to ensure poshial draimge. H would be appreciated, if could provede good nutrition and help in diet monitoring. Paracetamal is prescrived for pain during pain.
