Dear Mrs Adison,
I am writing to request home visits for two weeks to maintain Mrs Adam ‘s condition following her discharge today, she was admitted on 21 August, 2021 due to a fractured hip.
On 21/07/2021, Mrs Adam had a fall at home while walking and sustained left hip fractured, for which she underwent left total hip replacement surgery.
Postoperatively, she had delirium for five days and gained confusion regarding time, place and person.she was commenced on dalteparin 5000iu and physiotherapy in order to improve her mobility.on subsequent days, she made a good recovery and was able to walk with the help of stick and was performing activities of daily living.Her wound dressing were done regularly and slight sweeling on hip region was noted. She was encouraged for self administration of deltaparin, which she was taking under supervision.
Mrs Adam is a retired teacher and lives alone. She has had blurred vision since 2018 .
Given the above, it would be greatly appreciated if you could monitor her medication regimes and encourage her for exercise. Additionally, observe her wound for any swelling and pressure ulcer. Please note, her wound needs to be redressed on 06 September , 2021 and ceased her dalteparin on 17 August. Apart from this, her medication should be revival on 22 September and encourage her for anti embolism stockings until 06 September.
