Re: Ms Betsie Anderson,aged 89
I am writing to update you regarding the current health status of Ms Anderson, who is recovering from urinary tract infection,requires continuing care and support ,following her discharge today.
Ms Anderson was admitted on16 th of this month,with symptoms suggestive of urinary infection.On admission, he has low-grade fever,drowsiness and confused.In addition,he has weight loss of six pounds in one weak.Subsequently,she underwent laboratory investigations which showed bacterial infections.She was managed with intravenous antibiotics and acetaminophen 650 mg.
During hospitalization, Ms Anderson’s daily assessment include orthostatic blood pressure and loss of consiousnes, which needs to be continued.Physiotherapy has been initiated .She was assisted with activities of daily living and frequent toileting with proper hygiene.
It would be beneficial if you could arrange a follow-up visit will be scheduled wih Dr Alex rosw after two weeks with repeat urine analysis.Kindly,monitr her for any signs of dehydration .Please note,it is important to provide walker,cane and bedside commade for her speedy recovery.Her medication list is enclosed with this letter.
