Re: Sheila Cartwright, 69 years


Dear Penny


Mrs Sheila Cartwright who has been diagnosed with unstable diabetes mellitus and small infected ulcer on her left foot requires medical and social needs following discharge today.


On 14 April 2017, she was hospitalized with the above mentioned complaints for which tablet Glibenclamide 5mg, Metformin 800mg t.d.s and Amoxycillin 125 mg orally b.d were provided. Daily dressing with saline was done. Additionally, wound margins, signs of complications and healing process were monitored. She is known case of non-insulin-dependent diabetes mellitus since 9 years.


Mrs Cartwright worked in a administrative position and retired at the age of 67. Furthermore, she receives small pension for her daily living. Socially, she has no relatives or close friends and shows no outside interests. She has been living with all alone in three bedroom home. However, her alcohol intake has increased and dietary quality has decreased since her retirement.


Based on the aforementioned circumstances, medication compliance, blood sugar levels, and alcohol intake to be monitored. Moderate exercise programme to be encouraged along with social activities. It would be beneficial, if suggestions regarding establishment of income-producing activity is provided to her. Should you have any further queries, please do not hesitate to contact me.
