Dear Mrs Adison,
Re: Mrs Margarat Adam, aged 75 Years
I am referring Mrs Adam, who underwent a Left total hip replacement surgery, requires ongoing care and support, following her discharge today.
On 22 July 2021, Mrs Adam presented to hospital followed by a fall at
home. After the assessment, an x-ray investigation was performed which revealed a fracture of the left hip acetabulum. Postoperatively, she has been commenced on tablet delirium for next 5 days. During the hospital stay, her pain and constipation were managed by injection paracetamol 1 gm, tablet dulcolax 10 ml respectively. Additionally, she was seen by a physiotherapist and mobility support has been provided twice daily. On 06 August 2021, excellent performance of the wound has been noticed but little edema has been noted on her hip region.
Mrs Adam is a widower, retired teacher and lives alone. She was very active and independent before. She has had macular degeneration and osteoporosis since 2018, 2020 respectively. Therefore, she is on tablet alendronic acid 70 mg, calcium, and vitamin D for osteoporosis. Also, she is on tablet deltaparin 5000 IU hence, close monitoring of adverse effects of the medication will be required.
Pertaining to the above-mentioned information, kindly take over the case of Mrs Adam and provide continuous assistance and observation for her medication compliance. She is scheduled for hospital follow-up on 06 September 2021 for wound care and anti-embolism stockings. The community nurse visit for every two weeks would be profoundly beneficial.
