Re: Sona Mittal, aged 2

Thank you for seeing Sona Mittal, whose symptoms included cough, weakness, fever, shortness of breath, sunken eyes and dry skin and is being malnourished. She is being referred to you for expert advice and management for nutritional requirements following the discharge today.

During hospitalisation, antibiotics along with inhalation and hot water steaming were provided. Nutritional requirements were taken care since the day of admission. Excellent progress has been observed in eating, running and physical health as well .

In addition, speech, growth were affected. Hence, improvements in speech after treatment has been noticed. Mother is advised to prepare nutritional meals for Sona’s health condition. Nutrient rich diet is highly necessary at this moment.

Sona lives with her mother named Sangeetha Mittal, who is an unemployed school teacher and moody by nature. Father lives in another country, unable to come. Sangeetha is also dependent on social help and only care giver at home.

Based on the aforementioned circumstances, your expert nutritional assessment and appropriate management to fulfill the nutritional demands of Sona’s health condition would be profoundly beneficial. In case any additional information is required, please do not hesitate to contact me.
