Re: Ms Margaret Helen Martin, aged 81
Ms Martin, who had a successful angioplasty today for the coronary artery disease and angina, requires the assistance of community nurse following discharge tomorrow.
she has coeliac disease and also overweight with BMI 29.5, a contributing factor for overweight is her sedentary life style. lives alone in her home and hardly leaves or interacts socially which can lead to isolation.
Her current medication aspirin and plavix for coronary artery disease, Betaloc and Tritace for hypertension and lipitor for hypercholestermia. Monitoring of medication compliances highly recommended.
Ms Martin feels that she may not be able to cope with this situation alone at home therefore regular family visits are essential to prevent feeling of isolation. Instructions has been given for not to lift heavy items for 12 weeks. physiotherapist will support her for daily exercises.
Currently she receives weekly meals on wheels and a Dietitian will supervise her for dietary habits to maintain good health. Her referred doctor will also visit for monitoring of her life at home.
Nevertheless your assistance required to monitor her diet, exercise and bruising of catheter insertion site for any infection.
