Dear Ms Bruin,

Re: Gerald Baker Aged:79


The purpose of this letter is to refer Mr Baker ,who underwent left total hip replacement and had osteoarthritis .He requires continuous care and assessment and is ready to be discharged ,your ample care is highly recommended.


With regards to his medical history ,in 1985 he had Collin fracture and hypertension in 1989 respectively, for which he was commenced on aspirin 100mg and ramipril 5mg.

Socially he lives in a nursing home from past 4 years and working as a radio engineer, presently an aged pensioner His sister is supportive and visits regularly.


On observation, daily dressings are performed in the incision site .Weight bearing with use of wheeler waker ,and he walks without difficulty.


In view of the above, it would be greatly appreciated if you provide ongoing monitoring for anaemia, and continue the exercise program. Please note to preserve the skin integrity.
