Re: Ms Sheila Dyer, aged:72


Thank you for seeing Ms Sheila Dyer, who was presented with left-side Ischemia stroke, MCA, dyslipidemia and hypertension. She requires continued care in your facility.


On 10 March 2019,Mrs Dyer was treated with tissue plasminogen activator and supplemental oxygen. Some muscle in right arm and upper extremity are weak due to that difficult to manage daily activities. Aphasia and apraxia are present. Nutrition education regarding diet are recommended. A strengthening and stretching exercises have been provided from physiotherapy. To improve aphasia and apraxia language therapy are recommended.


Ms Dyer was retired officer worker and have mild depression after her husband's death. Current medications are aspirin 75mg and enalapril 25mg are daily; dipyridamole 200 mg, metoprolol 50 mg and dabigatran 75 mg twice a day. Atorvastatin 40mg twice a day or at night but LDL<70 mg/dl has to take.


Continuous monitoring is highly recommended for ensuring medication compliance. Monitor blood pressure goal below 140/90mmhg and lower cholesterol LDL<70mg/dl are provided. Motivation regarding leisure and social activities are recommended.


Based on the aforementioned circumstances, your immediate observation and appropriate management of Ms Dyer's health and condition would be profoundly beneficial. In case any additional information is required, please do not hesitate to contact me.
