Dear Sir/Madam
Re: Mr. Alfred Billy, age 52
I am referring Mr. Billy, who was diagnosed with basal cell carcinoma on the neck, for ongoing care and support following the discharge today.
On 21 March, Mr. Billy underwent basal cell carcinoma removal and a biopsy was taken for pathological investigation. Subsequently, the pain was managed with panadein forte 500mg. Mild wound infection, reactions in the suture area, and low skin integrity were observed. However, the healing process was normal, and no further signs of swelling were noticed.
Mr. Billy is a truck driver and lives with his wife. The habit of alcohol consumption for the past 30 years and regular smoking of cigarettes was reported. There is no history of any medical conditions and drugs were noted. Excellent progression has been observed.
Based on the aforementioned circumstances, daily observation and management of Mr. Billy's health conditions would be profoundly beneficial. In case you require any additional information, please don not hesitate to contact me
Yours faithfully
