Re: Mr Gerald Baker, aged 79
I am writing to refer Mr Baker, who underwent a left total hip replacement back into your care. He is being discharged today .
Mr Baker was admitted to the hospital on 03/09 /2018 for the above-mentioned surgery . During hospitalization he was commenced on tablet ramipril 5 mg in the morning and two tablet of Panadeine forte four times a day .Postoperatively, he was recommended on aspirin 100 mg in the morning .
Mr Baker was mobilized with the aid of wheelie Walker . He was given a transfusion of three units of packed red blood cells due to the Drop of hemoglobin levels to 72 g/dl. Please note that ,he was disoriented during recovery due to an anaesthetic effect .
Upon discharge it would be greatly appreciated if you could monitor his hemoglobin level in order to prevent anaemia. Kindly monitor his medication compliance .Please encourage him to continue the exercise program .Please note ,the hospital will provide the required equipment including Walker and pillow .His staples should be removed in the two weeks and a follow-up tests is needed.
