Dear Nurse,

Re: Mr Alfred Billy, aged 52

This letter is being written to refer Mr Alfred Billy, who is recuperating from basal cell carcinoma, requires further assessment and appropriate management following his discharge today.

During hospitalization, skin biopsy was performed to Mr Baker for pathological study. Basal cell carcinoma surgery was performed. However the wound healing was prolonged due to infection ,breakdown of skin and suture reaction. Excellent wound healing is process is noticed without any persistent feeling. For pain relief panadin forte 500 mg is commenced.

Additionally, no complaints was reported on the time of discharge. Also, pain is noticed but completely subsided.

It would be greatly appreciated if you could perform daily observations for Mr Billy. Please monitor his medication compliance. Continuation of medication is required for another week.
