Dear Ms Bruin,
I am writing to update regarding Mr Baker who was admitted on 03/019/2018 due to left hip replacement surgery .He requires further care and management from your facility following his discharge today.
Post operatively, Mr Baker has been commenced on aspirin 100mg in the morning, ramipril 5mg in the morning and panadeine forte four times a day for hypertension and pain respectively.Apart from this, daily dressing was done on the incision site.He was commenced on range of motion, stretching and strengthening exercises along with occupational therapy.
In the recovery period, he gained good mobility and was weight-bearing with the help of a Wheelie walker. He was able to walk length of ward without difficulty.His hb level dropped to 72g/ dl after surgery for which three units of packed red blood cells were transferred. After that his hb level was stable ; however he still needs monitoring for anaemia.
Given the above, it would be greatly appreciated if you could monitor his medications and preserve the skin integrity. In addition, continue his exercise program.
Upon discharge, he is provided with a wheelie walker and a wedge pillow which he needs and social worker has arranged a toilet raiser from the local medical supplier for two weeks. Please note, his staples needs to be remove in two weeks and also follow up FBE and UEC tests at city Hospital clinic.
