Dear Ms Ponsford,
This letter is being written to update regarding Mr Ramamurthy who was admitted on 04/02/2014 and was diagnosed with pneumonia.He is being discharged today back to your facility and requires monitoring on his diet and health.
On admission, he was experiencing a fever, rigors and suffered with acute shortness of breath, wheezing and sleeplessness. He was having chest and abdominal pain as well due to prolonged persistent cough. He was very weak and was not able to walk without a frame .He was assisted with activities of daily living such as showering and dressing etc.
On today's assessment, he gained independency and can walk slowly without assistance and can have shower and use the toilet independently.He still has some chest and abdomen pain due to dry cough. He has gained weight and was reviewed by a dietitian.
Given the above, it would be greatly appreciated if you could monitor his condition and please encourage him for oral fluids and proper nutrition.please encourage him for chest and physio such as deep breathing and coughing exercises . Kindly ensure that he sits up rather than lies down to ensure postural drainage. Paracetamol can be given if necessary for pain and keep him warm.
