Re: Baby Alicia Nesbitt, DOB: 22 April 2016
Alicia, Who was diagnosed with low birth weight and opioid dependence, requires regular monitoring and follow-up care after being discharged today.
During hospitalisation, both Alicia and her mother, Ms Poole, completed their heroin withdrawal without any complications. Although, Alicia’s weight got increased from 2.0kg to 2.3kg, decreased appetite has been noticed. Apart from this, poor bonding between mother and baby has been observed. Therefore, Ms Poole needs to be prompted oftenly for baby care.
Ms Poole, has been remained heroin dependent for two years and has also worked as a sex worker. Additionally, she got separated from her partner due to domestic violence; however, her mother is very supportive. Along with this, social services have been involved and they approved the discharge of both to live with Alicia's grandmother.
In view of the above, it would be greatly appreciated if you could arrange for daily visits by your team until Alicia’s weight and feeding becomes stabilised. In addition, provision to ensure safe environment for baby would be beneficial. Adequate monitoring for Ms Poole’s coping and psychological state and liaison with the drug and alcohol team to provide integrated support would be needed. Also, Alicia’s mother and grandmother should be educated appropriately for infant care. Doctor’s notes has been attached with this letter.
