Re: Mr Gerald Baker, aged 79
Mr Gerald Baker has undergone a left total hip replacement surgery, requires continuing care after being discharged today.
Mr Gerald Baker has been suffering with hypertension since 1989. Aspirin 100 mg mane, Ramipril 5mg mane and Panadeine forte two qid prn were commenced.
During hospitalization, daily dressing were provided. Assistance regarding range of motion, stretching exercises were provided. Occupation therapy was given. Ambulation is possible wheelie - walker without any difficulty. Low hemoglobin level was recorded post operatively, for which three units of packed red blood cells were administered. Now hemoglobin is stable; however, needs to be monitored. Also, follow-up for FBE and UFC tests at City hospital clinic was scheduled.
Following discharge monitoring of medications would be highly appreciated. Skin integrity needs to be preserved and exercise program must be continued. Wheelie- walker and pillows has been provided by us, and toilet raiser will be arranged by the social worker from the local medical pharmacy for two weeks.
