Re:Mr Christopher Fort, aged 49 Years


I am writing to request home visits for Mr Fort, who requires ongoing care and management from your facility. He underwent Percutaneous translational coronary angioplasty and is being discharged today.


On 20 February 2020, Mr Fort was admitted to hospital with chest pain which was radiated to the shoulder and breathing issue along with other minor ailments. After assessment, ECG and Blood investigation were performed which revealed Myocardial infarction. Post operatively, excellent progress of his condition has been observed. During hospital stay, he was evaluated by a dietician and instructed to avoid oily and carbohydrate rich foods. Additionally, information was provided to him about the intake of green leafy vegetables and fruits.


Me Fort has had Hypertension and Diabetic for the past 8 years, 9 years respectively. Therefore, he is on tablet Nicara 5 mg once daily and human mustard twice daily. He has been smoking and drinking for the last 10 years and eats oily as well as sweeter creams. Additionally, he was not engaged for any physical activity. All medication list is enclosed with this letter for your kind reference.


Pertaining to the above mentioned information, kindly take over the case of Mr Fort and provide appropriate care, monitor his medication compliance, and give instruction regarding dietary control. It would be highly appreciated, if you could monitor his blood pressure and blood sugar levels as well as provide instruction regarding daily exercise. Please note that, he is due for hospital follow-up on 3 March 2020, and instruct him to avoid hard exercise for the next couple of weeks.
