Re: Ms Sheila Cartwright, aged 69


Dear Nurse,


Ms Sheila Cartwright, who was diagnosed with unstable diabetes mellitus and an infected left foot ulcer, requires ongoing care and appropriate management at your facility, following the discharge today.


On 14 April, Ms Sheila Cartwright was admitted with the aforementioned diagnosis. Consequently, glibenclamide 5mg once a day, metformin 800mg thrice a day and amoxycillin 125mg thrice a day were commenced. Additionally, daily dressing changes have been provided, have to be continued.


Ms Cartwright lives alone, with a small post retirement income. Despite no relatives and friends, lack of interest in outdoor activities has been noted. Excessive alcohol consumption along with decreased dietary quality has been noted.


Following discharge, regular monitoring for ensuring medical compliance and blood glucose levels would be greatly appreciated, as difficulty with self-administration of hypoglycaemic agents has been reported. Apart from that suggestions related to establishment of income-generating activities would be beneficial. Moderate exercise programme and social activities should be encouraged. Continuation of glibenclamide is required. A review after antibiotic course completion is scheduled.
