Re:Mrs.Jasmine Thompson,aged 75
I am referring Ms.Thompson,who have undergone a right total shoulder replacement on 11July 2017.She requires your support and quality care following the discharge today.
Throughout the hospitalisation,she was hemodynamically stable followed by uneventful post operative recovery. The incision and drain site was clean and healthy. Post operative pain management was well supervised.Proper analgesics we’re commenced along with cold compression and a shoulder brace applied for 4 hours a day.
Gradually physiotherapy was initiated as per her recovery requirements,which to be continued at the outpatient for 2 weeks in addition with plus hydrotherapy for 1 week with prior appointment.Due to the healing process,lifting to be strictly avoided.Rather,the right arm to be rested on a sling for 4 weeks. And also post operative staples to be removed on 10 day with a follow up appointment in the Ortho joint replacement outpatient department.
Mrs.Roberts highly requires assistance to perform daily living activities and administration of prescribed DVT prophylaxis,Clexane subcutaneous for 4 days.
Based on the aforementioned circumstances, uninterrupted assistance and good care would be profoundly beneficial for Mrs.Roberts’s health condition. In case, you require any additional information, please do not hesitate to contact me.
