Re: Mrs Sheila Dyer, aged 72
Mrs Dyer is being discharged back into your facility for continuing care and monitoring. She was diagnosed with left-sided ischemic stroke involving middle cerebral artery. Currently, she is recuperating well.
As you know, Mrs Dyer was admitted to the hospital 2 months ago with ischemic stroke and severe hypertension. Therefore, she was commenced forthwith on tissue plasminogen activator and supplemental oxygen. Apart from this, the medication list will be attached with this letter.
During hospitalization, Mrs Dyer has developed upper extremity weakness leading to problem with dressing, showering and toileting; therefore, regular physiotherapy is required with stretching and strengthening exercises. Language therapy has been provided for sustaining aphasia and apraxia which has to be continuous following her discharge. Her BMI has increased currently to 38.
In view of the above details, it would be profoundly beneficial if you could monitor mrs Dyer's medication compliance as well as encourage her with leisure and social activities. Kindly note, she needs dietitian's advice to reduce BMI to normal. Please note, she has a consultation with neurologist on 22 May.
