Dear Ms Dsouza

Re: Mrs Angelina Jolly, aged 55

Mrs Jolly is being referred to you for continuing care and monitoring postoperatively for 3 days. She underwent tonsillectomy. currently she is recuperating well.

Mrs Jolly was admitted on 16 February 2019 with complaints of severe joint and throat pain, headache, fever, cough and breathing difficulty. Vital signs were abnormal. Clinical assessment and laboratory diagnosis revealed enlarged tonsils, red eyes, coated tongue and raised ASO titre, WBC and ESR. Consequently, her diagnosis was confirmed as hemorrhagic tonsillitisand rheumatoid arthritis. Therefore, she was treated with antibiotic paracetamol side replacement therapy and betadine gargling. Apartt from this, she is a known case of insulin dependent diabetes mellitus, hyperlipidemia and hypothyroidism.

Socially, Mrs jolly lives with her husband and two children. She consumes junk foods and alcohol occasionally. Additionally, she is a chain-smoker.

Upon discharge, Mrs Jolly requires continuation of medication and gargling. Please note, she needs rehabilitation for smoking. Kindly, report if heavy bleeding occurs. Worth to note, she needs hospital admission back after 3 days.

Based on aforementioned circumstances, it would be highly beneficial if you could provide necessary care following mrs Jolly's operation.

Should you require any further information, please do not hesitate to contact me.

Yours sincerely
