Re: Mr Gerald Baker, aged 70


Dear Nurse,


Mr Gerald Baker, who underwent surgery for a left total hip replacement, requires ongoing care and appropriate management at your facility, following the discharge back today.


After postoperatively, the doctor recommenced aspirin 100mg, ramipril once a day respectively, whereas Panadein Forte was prescribed when necessary. The daily dressing at incision site was performed. Additionally, moderate exercise programme was provided, the stitches will be removed at City hospital on 21 September. Excellent performance was noted. Therefore The walker was used to bear up the weight of his body. Regular monitoring for anaemia is highly recommend.


Following discharge, regular monitoring is suggested for ensuring medical compliance. Moderate exercise programme should be continued. His skin integrity should be preserved. The walker and a wedge will be provide by the hospital for atleast two weeks.
